Qualifications

• Bachelor’s degree in a related field
• Two years of experience preferred
• Excellent writing skills required
• Valid driver's license and reliable transportation

Responsibilities

• Write and implement plans of care approved by the consulting agency clinician.
• Write contact notes, monthly reports and discharge summaries.
• Provide in-home services including parenting education, trauma education, advocacy, school issues, and kinship support.
• Attend trainings and participate in coordinating treatment plans and meetings.
• Visit client homes to ensure plan compliance and support groups for families.
