Qualifications

Responsibilities

• Provide safe, evidence-based nursing care across the care continuum.
• Administer medications, perform treatments, and document care per policy.
• Collaborate with the interdisciplinary team to coordinate care and discharge planning.
• Identify changes in patient condition and initiate appropriate interventions.
• Educate patients and families using teach-back for engagement and safe transitions.
• Follow safety, infection prevention, and HIPAA-compliant practices.
• Document care plans in EMR systems and ensure accuracy.
• Participate in quality and patient safety initiatives.
