Job summary

Clarvida seeks an ACT Nurse (RN or LPN) in Mandeville, LA to join our Assertive Community Treatment team serving adults with severe mental illness. This part-time role provides community-based services to support recovery, stability, and independence.

The position involves in-home visits, coordination with medical and psychiatric staff, medication management, and rotating on-call duties. A company vehicle is provided for client transport as needed.

Qualifications

• Active RN or LPN license in Louisiana
• Experience with severe and persistent mental illness and/or substance use disorders preferred
• Psychiatric nursing experience preferred
• Valid driver's license in good standing and current automobile insurance

Responsibilities

• Provide nursing services as part of the ACT multidisciplinary team
• Meet with clients in homes, community settings, and occasionally the office or via phone/Zoom
• Conduct basic health assessments, including vitals and labs
• Support medication management and facilitate long-acting injectable medications
• Coordinate care with medical providers and psychiatric staff
• Support clients with severe mental illness and/or substance use disorders
• Participate in rotating on-call schedule (roughly one week every 7–10 weeks)
• Provide client transportation using a company vehicle as needed
• Maintain accurate, timely, and compliant documentation
