Qualifications

• Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Child Development, Special Education, or related field preferred.
• Experience providing case management, family support, parent education, community outreach, social services, or advocacy preferred.

Responsibilities

• Provide comprehensive, individualized case management to assigned families.
• Conduct intake procedures and assessments to identify client strengths, needs, barriers, risks, and service goals.
• Develop, implement, monitor, and update individualized service plans with clients and families.
• Maintain an assigned caseload and meet grant-required caseload, contact, service, and outcome goals.
• Maintain regular contact with clients through home visits, office appointments, calls, and virtual meetings.
• Advocate for families and connect them with appropriate resources and services.
