Qualifications

• RN degree required and Minnesota licensure preferred.
• Strong communication skills and teamwork.

Responsibilities

• Recommend and implement protocols to close gaps in preventive and chronic disease management.
• Access EHR and registries to identify care gaps at patient level.
• Review appointments and identify patients for follow-up care.
• Prepare reports listing open gaps and due services.
• Communicate with practice managers to improve quality scores.
• Assist IT and health plans with audits and documentation.
• Help create policies and workflows to embed quality work in teams.
• Educate providers and staff on reports and workflows for gap closures.
