Journey Hospice is seeking a Per Diem Palliative Nurse Practitioner to provide medical examinations and palliative visits for hospice patients in Atlantic County. This per diem role offers flexible scheduling with no guaranteed hours.
Under supervision with the Hospice Medical Director, you will document visits for Medicare Hospice Benefit eligibility after six months of hospice care and coordinate with the interdisciplinary team to deliver compassionate care.

We are seeking a Per Diem Palliative Nurse Practitioner to work with our hospice patients in Atlantic County. This position is Per Diem, therefore no guaranteed hours, but offers the flexibility to work with your schedule!
The Nurse Practitioner will work in accordance to prescribed policies and procedures of the organization including all applicable state and federal regulations and under the general supervision and in conjunction with the Hospice Medical Director, the Advance Practice Nurse is responsible for performing patient medical examinations and palliative care visits relative to the face-to-face encounters required in order to certify patient’s eligibility for the Medicare Hospice Benefit after six (6) months of hospice care. Documents visits in the patient’s clinical record and the face-to-face encounter on required forms. Provides medical care services and palliative care intervention for hospice patients on an as-needed basis. The Advance Practice Nurse communicates effectively with the Medical Director, Administrator and the Hospice Interdisciplinary Team.
