Job summary

Anew Hospice in Lafayette, IN is seeking a Palliative Nurse Practitioner (NP) to join a supportive, home-based care team. This PRN role offers flexible scheduling and the opportunity to manage your own time while delivering high-quality palliative care under physician supervision.

The NP will perform comprehensive assessments, develop care plans, and coordinate with the interdisciplinary team to meet patient needs. Competitive wages and mileage reimbursement are provided.

Qualifications

• Must be licensed in state and CPR certification as required by organizational policy.
• Master's degree with a minimum of one year NP experience in palliative/hospice care; or be eligible for palliative care certification.
• Pediatrics experience or training in developmental stages and needs of infants, children and adolescents.
• Certification in a specialist area (e.g., Hospice and Palliative Nursing CHPN) preferred.
• Excellent observation, verbal and written communication, problem solving and nursing skills.

Responsibilities

• Provide medical care and manage terminal illness in compliance with state Nurse Practice Act.
• Complete history and physical examinations and document findings in the medical record.
• Develop individualized care plans, prescribe medications within licensure, and coordinate hospice care services.
• Review and oversee patient care in consultation with the palliative care team and other practitioners.
• Ensure symptom control through interdisciplinary collaboration.
• Review and develop treatment protocols and propose interventions based on current literature.
• Consult with collaborating physician when the plan of care falls outside standard practice.
