The ideal candidate holds a Master's in Nursing, Indiana NP licensure, and experience in palliative or hospice care.

Qualifikationen

• Master's degree in Nursing with NP certification.
• Current Indiana nursing license and CPR certification.
• A minimum of 1 year NP experience with palliative or hospice care; certification is preferred.
• Pediatrics experience or training is preferred.
• Hospice/ palliative care certification (CHPN) preferred.
• Excellent observation, verbal and written communication, problem-solving skills.

Aufgaben

• Provides medical care, palliation, and manages terminal illness in line with the Nurse Practice Act.
• Performs history and physical examinations, orders/tests, diagnoses health conditions, and documents findings.
• Develops individualized care plans, prescribes medications within licensure, coordinates hospice care services.
• Reviews clinical information and coordinates patient care with the interdisciplinary team.
• Ensures symptom control through collaborative care planning with team members.
• Develops treatment protocols and proposes options based on current literature.
• Consults with the physician or leadership when plan falls outside standard practice.
