Concordia Visiting Nurses is seeking a "Palliative Licensed Practical Nurse"for a full time position which will cover our patients living in the Brentwood and Baldwinservice area. No weekends! No Holidays! No on calls! Graduate nurses are welcome to apply!Your patients, your schedule.

POSITION SUMMARY

• The Palliative Licensed Practical Nurseis a licensed healthcare professional with advanced medical knowledge who administers skilled nursing care to patients requiring intermittent visits in their homes. She/he provides complex care to the patient and coordinates necessary support services. These nursing services are provided to patients in accordance with a physician’s plan of care. RNs who serve as primary nurses utilize case manager approach in assessing, implementing, coordinating, and evaluating client care.

QUALIFICATIONS FOR THE LICENSED PRACTICAL NURSES POSITION:

• Graduate of state-approved school with course in pharmacology
• Current license in the State of Pennsylvania
• Minimum of two [2] years experience in patient care preferred
• Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state.
