Concordia Visiting Nurses is a Medicare-certified, state-licensed, Medicaid-approved home health agency operating in nine Western Pennsylvania counties. We are seeking a Palliative Licensed Practical Nurse for a full-time position covering patients in the Brentwood and Baldwin service area. No weekends, no holidays, no on-call duties! Graduate nurses are encouraged to apply. You will have your own schedule and patient assignments.

Position Summary

The Palliative LPN is a licensed healthcare professional responsible for administering skilled nursing care to patients requiring intermittent home visits. They provide complex care, coordinate support services, and work according to a physician’s plan of care. RNs may serve as primary nurses using a case management approach.

Qualifications
