Position Summary

The Palliative Licensed Practical Nurse is a licensed healthcare professional responsible for administering skilled nursing care to patients requiring intermittent home visits. The nurse provides complex care, coordinates support services, and follows a physician’s care plan. RNs serving as primary nurses utilize a case management approach to assess, implement, coordinate, and evaluate patient care.
