Qualifications

• Two (2) years combined RN experience in an acute care setting or case management.
• Completion of an accredited RN training program that allows graduates to take the RN license exam.
• Registered Nurse License (California) and Basic Life Support.
• Demonstrated ability to utilize/apply utilization review, discharge planning and case management principles.

Responsibilities

• Coordinate and screen admissions and continued stays with an MD.
• Recommend alternative levels of care when medical necessity for inpatient care is not met.
• Develop and coordinate discharge plans with patients, families and community resources.
• Coordinate and monitor utilization management, transfer coordination and required authorizations.
• Educate and collaborate with the multidisciplinary team to ensure safe, cost-effective care.
