Job summary

Ennoble Care is a mobile primary care, palliative care, and hospice service provider operating across multiple states. We seek a dedicated Transitions Specialist to guide patients through the palliative continuum, from referral and eligibility screening to timely transitions into hospice or other suitable levels of care.

The role emphasizes early identification of suitable patients and ensuring goals-of-care conversations occur at the right moment.

Qualifications

• Candidates may have an LPN, RN, or Social Work background with direct clinical experience in hospice or palliative care.
• Strong organizational skills, phone presence, and proficiency with Microsoft Office and EMR systems are essential.
• Must be a positive, adaptable team player in a fast-paced environment.

Responsibilities

• Review incoming referrals, confirm eligibility, and tag cases in Zoho to prioritize urgent needs.
• Own transitions from palliative care to hospice, coordinating handoffs between providers and facilities.
• Complete care plans, document communications, and support monthly summaries and reports.
