Job summary

TriHealth seeks a Social Worker in Hospice Care to support patients and families in a compassionate, patient-centered manner. The role involves independent field-based work, collaboration with nurses, physicians, and caregivers, and ensuring comfort, dignity, and continuity of care across settings.

The ideal candidate will have a Master’s in Social Work or related field, an Ohio licensure, and at least some clinical healthcare experience.

Qualifications

• Master's Degree in Social Science or Social Work is required.
• Ohio Licensed Social Worker is required.
• Up to 1 year clinical healthcare or community social work experience is required.
• Computer literate.

Responsibilities

• Applies root cause analysis to readmissions and works with the health care team, patient, and family to resolve the issues.
• Communicates with the multidisciplinary team to advise the plan and patient response to the plan; counsels caregivers.
• Develops and implements plan of care based on assessment including counseling or crisis intervention; facilitates linkages to resources.
• Promotes the profession of social work and provides education and on-call services as needed.
• Responds to referrals and assesses psychosocial and discharge needs of patients and families.
