Job summary

TriHealth Hospice of Cincinnati/Hamilton’s North Region seeks a Social Worker to support patients and families, coordinating with nurses and physicians to ensure comfort, dignity, and continuity of care. The role covers hospital and community settings with on-call coverage during some periods.

The successful candidate will hold a MSW with a licensed social work credential, prefer hospice experience, and demonstrate strong communication and navigation of community resources to optimize patient

Qualifications

• Master's Degree in Social Work required
• Licensed Social Worker required
• Hospice experience preferred
• Proficient with EMR systems and patient data

Responsibilities

• Apply psychosocial assessment to readmissions and workflows with the health care team
• Counsel patients and families and coordinate with services and community resources
• Develop and implement care plans addressing psychosocial needs and discharge planning
• Educate and mentor staff on social work practices and regulatory requirements
• Respond to referrals and assist in transition between care levels
