Job summary

Qualifications

• Master’s degree in Social Work required.

Responsibilities

• Participates in Multidisciplinary Team assessments, care planning and reassessments.
• Maintains case management records in paper and electronic formats.
• Provides individual and family counseling and leads group counseling activities.
• Coordinates discharge planning and post-discharge follow-up.
• Advises staff on the impact of social, emotional and cultural factors on care.
