Qualifications

• MSW required and state licensure obtained prior to/s during supervision hours.
• Basic computer proficiency with word processing and spreadsheets.
• Strong written and verbal communication skills.
• Demonstrated ability to apply critical thinking skills.
• Caregivers visiting home/community require current BLS, valid driver’s license, auto insurance, clean driving record and reliable transportation.

Responsibilities

• Conduct psychosocial assessments and crisis planning as part of care management.
• Develop and monitor care plans addressing social determinants of health and community resources.
• Provide brief individual, group, and family therapy.
• Identify mental, emotional, and behavioral disorders and connect patients to services.
• Educate and advocate for patients and staff; facilitate safe transitions of care to the community.
• Collaborate with healthcare teams, insurers, and community providers for quality care.
• Lead improvement initiatives and monitor policy adherence; support advanced care planning.
