Qualifications

• MSW degree from an accredited institution.
• Current state licensure or licensure to be obtained prior to/supervision hours completion.
• Strong written and verbal communication skills.
• Basic computer proficiency (word processing/spreadsheets).
• Caregivers with home/community visits must maintain BLS and valid driving/insurance.

Responsibilities

• Assess patients for transition planning, mental health, and goals of care.
• Develop and monitor care plans, addressing social determinants of health.
• Provide motivational interviewing and therapeutic techniques for mental health support.
• Deliver brief individual, group, and family therapy and psychosocial assessments.
• Identify mental/behavioral disorders and connect to services.
• Educate and advocate for patient rights and safe transitions of care.
• Collaborate with healthcare teams, insurers, and community providers.
• Lead quality improvement initiatives and ensure policy adherence.
• Facilitate clinical goals of care discussions with patients, families, and teams.
