Job summary

CenterCare Hospice is seeking a compassionate Registered Nurse (PRN) to provide in-home nursing care in Pueblo, Colorado. The RN will assess patient needs, coordinate with the care team, and support families facing end-of-life decisions.

Qualified candidates hold an active RN license, CPR/BLS, and hospice experience. Responsibilities include medication administration, charting, and ensuring comfort and symptom management for patients in their residence.

Qualifications

• Must possess a Nursing Degree from an accredited program or be a graduate of an approved RN program.
• Must possess a current, unencumbered RN license.
• Must have CPR/BLS certification.

Responsibilities

• Provide nursing care in the patient’s residence in accordance with applicable standards and regulations.
• Assess patient and family needs and tailor nursing services accordingly.
• Communicate with physicians and care team to coordinate care.
• Educate patient and family on disease process, self-care, and end-of-life care.

We are dedicated to focusing on comfort and symptom management while improving the quality of life in an atmosphere of dignity, compassion and hope.CenterCare Hospice is looking to add a Pueblo Registered Nurse for PRN to their team.The ideal candidate will be compassionate, empathetic and able to dedicate themselves to our goal of providing the highest quality of care that focuses on the patient’s physical, emotional and spiritual needs.RN- Job Summary: Provide nursing care in the patient’s place of residency in accordance with current federal, state, and local standards, guidelines, and regulations that govern our organization, and as may be required by the Director of Nursing or designee.RN- Essential Job Functions:Administer services within the applicable scope of nursing practiceDetermine the scope and frequency of services needed based on the acuity of the patient and the family/caregiver needsAssess the ability of the caregiver to meet the patient’s needs upon admission and throughout careCommunicate with physicians, care team, patients and family members effectivelyConsult with and educate the patient and family/caregiver regarding the disease process, self-care techniques and end of life careRequisition and arrange for diagnostic and therapeutic services, as ordered by the physician, and in accordance with established proceduresFollow drug administration protocol, to include the disposal of drugs and narcotics as required, and in accordance with established proceduresPerform charting duties as required and in accordance with established charting and documentation policies and procedures and applicable state and federal regulationsReview care plans to verify that appropriate care is being rendered.
