Job summary

Qualifications

• Must hold a current RN license in Maryland
• Minimum of two years’ hospital experience
• BSN or related bachelor degree preferred
• Palliative care certification not mandatory to start, but should be completed within 2 years of employment
• Demonstrated authentic belief and acceptance of palliative care principles
• Licensure in the State of Maryland is required

Responsibilities

• Knowledge of advancing directives and assisting patients/families with completion
• Ability to work independently and as part of a multidisciplinary team
• Screen patients for palliative care needs
• Ensure effective communication with patients, caregivers, families and all disciplines for smooth transitions of care
• Maintain confidentiality of patient and departmental documents
• Communicate and teach stages of chronic conditions
• Effectively communicate end-of-life care and make hospice referrals when needed
