Job summary

ChenMed is seeking a compassionate RN to deliver supportive care within our Serious Illness/Palliative care program in Lakeland, FL. The role includes home and center-based assessments, coordination with PCPs and specialists, and proactive monitoring to prevent hospitalizations.

Responsibilities cover comprehensive assessments, symptom management, goals-of-care documentation, and collaboration with a multidisciplinary team. Willingness to travel locally up to 75% is required.

Qualifications

• Active RN license in the state of practice.
• Minimum 2 years’ clinical nursing experience.
• Experience in case management and palliative/home health care preferred.
• Willingness to travel up to 75%.

Responsibilities

• Patient Identification, Outreach, and Enrollment: outreach within 48 hours and first visit scheduling.
• Comprehensive clinical assessment including vital signs and symptom tools.
• Ongoing monitoring with cadence based on tiers and ESAS scores.
• Escalation to physician/NP via Door 1 or Door 2 when needed.
• Care coordination for post-discharge follow-ups and transitions of care.
