Job summary

Impact Health PC in Fairfield, CT is seeking a compassionate Palliative Care Nurse Practitioner to join our skilled nursing facilities team. The role offers a Monday–Friday schedule with no nights, weekends, or holidays, and provides access to physician-level consults for autonomy with support.

Compensation ranges from $115,000 to $200,000 based on experience, plus monthly bonuses, comprehensive benefits, PTO, 401(k), malpractice insurance, license reimbursement, CEU support, and ongoing

Qualifications

• Active Nurse Practitioner license (CT).
• Minimum 2 years post-certification experience.
• Experience in SNFs, hospitalist roles, ED or ICU preferred.
• Proficient with EHRs and clinical technology.

Responsibilities

• Deliver person-centered care using evidence-based practices.
• Collaborate with interdisciplinary facility teams to coordinate care.
• Assess patients, develop treatment plans, and manage follow-up.
• Provide ongoing follow-up and chronic care management in SNFs.
• Educate patients, families and staff; complete timely documentation.
• Ensure compliance with facility, state and federal regulations/protocols.
