Job summary

Qualifications

• Minimum five years of nursing experience required.
• Experience in oncology, hospice, geriatrics, or ICU preferred.
• Current national nursing certification (ANCC, NBCHPN, or AANP).
• Palliative care ARNP preferred.
• Knowledge of interdisciplinary team approach.

Responsibilities

• Facilitates clarification of patient and family goals of care.
• Facilitates access to appropriate support models of care (hospice, rehabilitation, and ongoing aggressive treatment).
• Assists with diagnosis, treatment and management of acute and chronic health conditions.
• Consults with palliative care physician and informs primary physician of recommended services.
• Educates facility staff, coworkers and the community on palliative care and services provided.
• Attends Palliative Medical Associates team meetings.
• Prepares and maintains accurate patient records and documents.
• Prescribes medications including controlled substances after collaboration with physicians.
• Orders diagnostics/treatments as needed after collaboration with physicians.
• Assists in development of clinical practice guidelines.
