Job summary

Impact Health PC in Waterbury, CT is seeking a compassionate Palliative Care Nurse Practitioner to join our growing healthcare team. The role offers a flexible Monday–Friday schedule with no nights, weekends, or holidays, and the opportunity for autonomous care with physician-level consults.

You will deliver person-centered care in skilled nursing facilities, collaborate with interdisciplinary teams, and develop treatment plans with follow-up management.

Qualifications

• Must hold an NP license and be in good standing.
• Minimum 2 years of post-certification experience.
• Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred.

Responsibilities

• Deliver person-centered care using evidence-based practices.
• Collaborate with interdisciplinary facility teams.
• Conduct assessments, develop treatment plans, and manage follow-up care.
• Provide ongoing follow-up and chronic care management in SNFs.
• Educate patients, families, and staff.
• Complete timely documentation (within 24 hours).
• Ensure compliance with facility, state and federal regulations/protocols.
