Job summary

Impact Health PC in Danbury, CT seeks a compassionate Palliative Care Nurse Practitioner to join our team delivering expert care in SNFs across the Northeast.

This full-time role offers a Monday–Friday schedule with no nights, weekends, or holidays, plus an earning potential of $115,000–$200,000 (based on experience) and monthly bonuses. Qualifications

• Active NP license in good standing.
• Minimum 2 years post-certification experience.
• SNFs, hospitalist, ED or ICU experience preferred.
• Proficient with EHRs and clinical technology.
• Strong communicator with collaborative mindset.

Responsibilities

• Deliver person-centered care using evidence-based practices.
• Collaborate with interdisciplinary facility teams.
• Assess, develop treatment plans, and manage follow-up care.
• Provide chronic care management in SNFs.
• Educate patients, families, and staff; document within 24 hours.
• Ensure compliance with regulations and protocols.
