Qualifications

Active RN/APRN license in the state of practice.
Nurse Practitioner or Clinical Nurse Specialist with prescriptive authority.
Current BCLS and CPR certification.
5+ years of clinical nursing experience.
Valid driver's license and reliable transportation.

Responsibilities

Conduct comprehensive palliative care assessments across physical, emotional, psychosocial, and spiritual needs.
Review medical history, coordinate care, and perform medication reconciliation.
Develop and implement evidence-based treatment plans for serious illnesses.
Provide education and support to patients, families, and caregivers.
Collaborate with interdisciplinary teams to ensure high-quality patient-centered care.
