Job summary

NJ Palliative Care Solutions is seeking a Nurse Practitioner or Physician Assistant to deliver inpatient palliative care at Monmouth Medical Center in Long Branch, NJ. You will provide consultations, lead care planning, and collaborate with physicians, social work, and care coordination teams.

Requirements include active NJ NP/PA license, DEA+CDS, 2+ years in palliative care, and board certification. Expect a Monday–Friday schedule, 28 days PTO, and strong back-up support.

Qualifications

• Active New Jersey NP/PA license.
• 2+ years of palliative care experience.
• DEA + CDS registration, or ability to obtain by start date.
• Palliative care certification (ACHPN/CHPN) preferred.

Responsibilities

• Provide hospital-based palliative care consultations and follow-up visits.
• Manage complex symptoms for patients with serious illness.
• Lead goals-of-care and advance care planning conversations.
• Partner with interdisciplinary teams to integrate palliative medicine into hospital workflows.
• Complete timely documentation aligned with clinical and billing standards.
• Assist with discharge planning and transitions to next settings.
