Presbyterian Healthcare Services is seeking an outpatient focused Palliative Care Nurse Practitioner or Physician Assistant to join our Santa Fe practice. The role includes clinical oversight in hospice and palliative care with a strong emphasis on patient goals and quality of life.

Qualifications

• Licensed NP in New Mexico with a Masters degree.
• AANC or AANP certification for NP.
• Certified Physician Assistant with NM board license.

Responsibilities

• Collaborates with colleagues in management of patients in outpatient settings.
• Provides care to patients with life-limiting illness, aligning treatment with patient goals.
• Delivers expert symptom management to maximize quality of life.
• Builds relationships with referring physicians and care teams to improve collaboration.
