Job summary

Anpex Health is seeking a Nurse Practitioner to provide primary and palliative care focusing on symptom management and goal-concordant care. You will lead comprehensive exams, discuss goals of care, and coordinate with an interdisciplinary team to ensure patient-centered transitions.

Qualifications

• MSN or DNP in NP program completed, licensed to practice as NP in MA.
• Experience in primary care, geriatrics or post-acute care preferred.
• Palliative care training and certification provided.

Responsibilities

• Provide primary and palliative care focusing on symptom management and quality of life.
• Lead goals-of-care discussions including advance care planning and code status.
• Review and update advance directives and POLST/DNR orders.
