Job summary

Anpex Health is seeking a Nurse Practitioner to join a mission-driven team delivering patient-centered care in a collaborative environment in New Jersey. The NP will provide both primary and palliative services with emphasis on symptom management, goals-of-care, and quality of life for patients with complex illnesses.

The role involves interdisciplinary teamwork, advance care planning, and transitions of care support, with a focus on comfort and dignity.

Qualifications

• MSN or DNP completed as NP.
• Experience in primary care or geriatrics preferred.
• Palliative care training provided.

Responsibilities

• Provide primary and palliative care focusing on symptom management, quality of life, and goal-concordant care.
• Lead goals-of-care discussions, including advance care planning and code status conversations.
• Facilitate advance directives, POLST/DNR orders, and end-of-life care preferences.
• Collaborate with the interdisciplinary team (IDT) weekly to review patient status, prognosis, and care plans.
