Job summary

Qualifications

• Master's (MSN) or Doctorate (DNP) Nurse Practitioner program completion.
• Experience in primary care and geriatrics preferred; palliative care training provided.

Responsibilities

• Provide primary and palliative care services focusing on symptom management and quality of life.
• Lead and document goals-of-care discussions, including advance care planning and code status.
• Manage pain and non-pain symptoms using pharmacologic and non-pharmacologic approaches.
• Review vitals and labs and adjust interventions or escalate to attending physician.
• Collaborate with the interdisciplinary team weekly to review patient status and care plans.
• Identify patients for palliative or hospice referral and coordinate transitions of care.
