Qualifications
- MSN or Doctorate (DNP) Nurse Practitioner program completion.
- Experience in primary care, geriatrics, or post-acute care preferred, but not required.
- Palliative care training and certification provided.
Responsibilities
- Provide primary and palliative care services with focus on symptom management and goals of care.
- Lead and document goals-of-care discussions including advance care planning and code status conversations.
- Facilitate advance directives, POLST/DNR orders, and end-of-life care preferences.
- Manage pain and non-pain symptoms with pharmacologic and non-pharmacologic approaches.
- Collaborate with the IDT to review patient status, prognosis, and care plans.
- Support transitions of care with emphasis on continuity, comfort, and patient-centered goals.
