Job summary

Impact Health PC in Middletown, CT seeks a Palliative Care Nurse Practitioner (NP) to provide compassionate, patient-centered care for individuals with serious illnesses in skilled nursing facilities. You will assess symptoms, develop treatment plans, and support goals-of-care discussions while collaborating with physicians, staff, families, and interdisciplinary teams.

• Experience in palliative care, SNF, hospitalist, ED, or ICU is desirable.
