Qualifications

• RN/APRN licensure in state of practice.
• Nurse Practitioner/CNS with prescriptive authority.
• BCLS licensure.
• Minimum 5 years of clinical nursing experience.
• Current CPR certification.
• Preferred: 1 year as APRN in Hospice, Palliative Care, or Oncology.
• Advanced practice palliative care certification (ACHPN).

Responsibilities

• Provides direct patient and family care through palliative care consultations.
• Applies evidence-based palliative medicine practices to patient care.
• Gathers information from medical records, family history, and caregivers for assessments.
• Performs comprehensive assessments, plans care, and evaluates outcomes.
• Encourages ACHPN certification after 2 years of employment or when eligible.
