Job summary

Casa de la Luz Hospice Palliative Care in Tucson, part of the Optum family, is seeking a Palliative Care Nurse to provide direct patient and family care through palliative consultations for patients with serious illness and a prognosis of one year or less who are not ready or eligible for hospice care. The role emphasizes ethical, evidence-based practice, comprehensive assessments and coordination of care.

Responsibilities

  • Demonstrates effective and ethical use of palliative medicine practices in patient care across physical, emotional, spiritual, psycho/social, ethical and safety criteria.
  • Formulates an effective plan of care by applying palliative care knowledge.
  • Gathers information from medical records, family, caregivers and conducts thorough patient assessments.

As the Palliative Care Nurse, you will provide direct patient and family care through palliative care consultations, ensuring specialty services for patients with serious illness and a prognosis of one year or less who are not ready or eligible for hospice care.

Primary Responsibilities:
  • Demonstrates effective and ethical use of approved evidence-based palliative medicine practices in the delivery of all patient
    care based on physical, emotional, spiritual, psycho/social, ethical, educational, safety and related criteria, appropriate to the ages of patients served.
  • Demonstrates knowledgeable clinical expertise by incorporating palliative care body of knowledge to formulate an effective plan of care.
  • Gathers pertinent information systematically from all sources, including medical records, family history, and caregivers, and thorough specialty physical, emotional, psychosocial, and spiritual clinical patient assessment.
  • ACHPN certification encouraged after 2 years of employment or when experience requirements are met.

Required Qualifications:
  • RN/APRN licensure in state of practice.
  • Nurse Practitioner/CNS with prescriptive authority.
  • BCLS licensure.
  • Minimum 5 years of clinical nursing experience.
  • Current CPR certification
Preferred Qualifications:
  • Minimum 1 year as APRN in Hospice, Palliative Care, or Oncology.
  • Advanced practice palliative care certification (ACHPN).

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $95,686 to $143,530 annually based on full-time employment. We comply with all minimum wage laws as applicable.
