Northwestern Medicine seeks a Social Worker LSW to join our patient-centered care team at our Chicago area facilities. The role supports psychosocial assessment, discharge planning, and resource coordination to improve patient outcomes.

The LSW will collaborate with an interdisciplinary team, maintain HIPAA compliance, and participate in performance improvement activities while guiding patients and families through complex care pathways.

Qualifications

• Master's Degree in Social Work from CSWE-accredited program.
• Licensed Social Worker (LSW) licensed in Illinois.
• Strong interpersonal and organizational skills.
• Self-directed with ability to manage a caseload.
• Analytical skills to analyze data, negotiate, and solve problems.

Responsabilités

• Perform psychosocial assessment with patient and family, considering social, emotional, cultural, mental health, and financial factors.
• Develop and implement an intervention plan with mutual goals and patient preferences.
• Provide crisis intervention and protective services as needed, including safeguards for vulnerable adults and children.
• Coordinate discharge planning, home health care, and linkage to community resources.
• Collaborate with interdisciplinary team and participate in rounds and case conferences.
• Engage in ongoing professional development and adherence to policies, HIPAA, and code of ethics.
