Job summary

JPS Health Network in Fort Worth, TX is seeking a Licensed Clinical Social Worker to join the Palliative Care team on day shift at JPS Main. The role focuses on psychosocial assessment, crisis intervention, counseling, and guiding patients and families through serious illness.

The ideal candidate holds an MSW and LCSW, is CPR certified, and collaborates with an interdisciplinary team to develop and implement care plans while advocating for patient-centered, culturally sensitive end-of-life

Qualifications

• Master’s Degree in Social Work from an accredited university.
• Licensed Clinical Social Worker certification by the Executive Behavioral Health Executive Council (EBH) (formerly Texas State Board).
• Current CPR Certification and must be maintained throughout employment; may be obtained within 120 days from hire.

Responsibilities

• Provides psychosocial assessments for adult/geriatric patients and families.
• Delivers crisis intervention, counseling, and support for emotional and spiritual needs.
• Collaborates with the interdisciplinary team to develop and evaluate treatment plans.
• Maintains electronic medical records accurately per JPS standards.
