Job summary

Tuesday Health is hiring a Licensed Clinical Social Worker (LCSW) to deliver patient-centered palliative care remotely with EST residency requirement. You will collaborate with an interdisciplinary team to assess psychosocial needs, conduct serious illness conversations, and contribute to advance care planning.

The role emphasizes compassionate, culturally responsive care and care coordination across settings.

Qualifications

• : MSW from an accredited program
• : Active LCSW license in applicable state(s)
• : Clinical experience in palliative care including advance care planning
• : Experience in interdisciplinary care model
• : Strong written and verbal communication
• : Proficiency with electronic health records and documentation

Responsibilities

• Conduct comprehensive psychosocial assessments addressing distress, caregiver burden, social determinants of health, and risk factors in serious illness.
• Interpret assessments to identify priority needs and interventions within the palliative care model.
• Conduct serious illness conversations and facilitate advance care planning documentation.
• Provide clinical recommendations and escalate to physicians using established protocols when safety or psychosocial concerns arise.
• Develop and document clinically appropriate interventions within individualized care plans.
• Participate in interdisciplinary rounds and care conferences to support coordinated, goal-aligned care.
• Deliver short-term counseling, psychoeducation, and emotional support to patients and families.
• Identify psychosocial barriers to care and support transitions across care settings.
• Document assessments, interventions, and care plans in the EHR and ensure data quality.
• Build therapeutic, trust-based relationships grounded in empathy and culturally responsive care.
