AdventHealth in Orlando, FL is seeking a licensed clinical social worker to join AHMG Palliative Medicine and Supportive Care. The role involves psychosocial assessments, counseling, care coordination, and collaboration with the multidisciplinary team.

You will support patients and families through transitions, bereavement, crisis intervention, and program development. Requires MSW, LCSW or equivalent, with 2+ years of experience; full-time day shift.

Responsibilities

• Performs psychosocial assessments and provides psychotherapy to patients and families coping with medical issues.
• Maintains comprehensive community resources and networks to assist patients and families as they transition back to the community.
• Collaborates with the multi-disciplinary team to document psychosocial determinants for safe discharges.
• Serves as a consultant and educator to the team regarding psychosocial aspects of illness, disability, and hospitalization.
• Facilitates staff emotional debriefing and ongoing support groups.
• Applies knowledge of local, state, and federal programs to assist with referrals and post-acute coordination.
• Participates in developing programs to support patients and families with complex medical issues.
• Attends physician rounds and forums to provide psychosocial input to the team.
• Provides counseling services, crisis intervention, stress management, and bereavement support.
