Job summary

Health First seeks a Hospice Licensed Clinical Social Worker (LCSW) to coordinate psychosocial care for assigned patients in collaboration with families and the care team. The role emphasizes assessment, planning, intervention and evaluation to optimize outcomes in a hospice setting.

Requirements include a Master’s in Social Work, Florida LCSW licensure, 3+ years of social work experience, CPR/AED HeartSaver certification, and valid driver’s license with an acceptable MVR.

Qualifications

• Master’s degree in Social Work (MSW) required.
• Florida LCSW licensure required.
• Three years of social work experience minimum.
• CPR/AED HeartSaver certification required prior to start.
• Driver’s license and clean MVR.
• Background check per Florida AHCA requirements.

Responsibilities

• Performs psychosocial assessment to identify risks and coordinates with the care team.
• Facilitates complex cases and makes referrals for safety concerns or abuse.
• Recommends interventions to support safe and efficient plan of care.
• Identifies transitional care needs (ALF/LTC, respite, inpatient transfer).
• Communicates options to patients, families, practitioners and staff.
• Assesses at-risk populations and safety planning, including Baker Act when needed.
• Documents education for patient/family on psychosocial care and outcomes.
• Initiates referrals to Ethics Committee, Risk Management or Legal services.
