AccentCare is seeking a Director of Palliative Care in the Milwaukee area to oversee inpatient hospice services across hospitals, SNFs, and inpatient units. You will lead clinical delivery, supervise physicians, and coordinate patient-centered care with the Medical Director and interdisciplinary teams.

The role emphasizes leadership, strategic planning, and high-quality symptom management for terminal illness. Hybrid schedule with occasional on-site facility visits is expected.

Qualifications

  • Licensed MD/DO in WI with training or experience in caring for chronically or terminally ill patients
  • Maintains active DEA license
  • Demonstrated ability to collaborate with physicians, facility staff and leadership
  • Strong knowledge of hospice and palliative care with emphasis on symptom management
  • Board certification/fellowship in Hospice and Palliative Medicine preferred

Responsibilities

  • Provide ongoing education and training to inpatient facility staff in coordination with clinical leadership
  • Participate in monthly leadership meetings with Medical Director and inpatient leadership
  • Support and advise physicians and patient care teams to ensure high-quality care delivery
  • Collaborate with attending physicians regarding patient prognosis and care planning
  • Oversee and coordinate patient admissions, discharges, and transitions within inpatient settings
  • Ensure continuity of care and effective handoffs between care teams during inpatient stays
  • Promote best practices in hospice and palliative care across all inpatient settings
  • Conduct periodic reviews of clinical care quality delivered by inpatient physicians
  • Make occasional on-site visits to hospitals for meetings and leadership needs

Title: Director of Palliative Care

Location: West Allis, WI

Schedule: 6-8 hrs/wk

Position Type: Hybrid

The Palliative Care Physician is responsible for overseeing the medical delivery of hospice inpatient services across various care settings, including hospitals, skilled nursing facilities, and hospice inpatient units. This role ensures high-quality clinical care, effective physician supervision, and strong coordination of patient-centered services in collaboration with the Medical Director and interdisciplinary teams.

What You Need to Know

If you meet these qualifications, we would love to meet you:

  • Must be a licensed Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) in the state(s) of practice, with training or experience in caring for chronically or terminally ill patients
  • Maintains active DEA license
  • Valid driver's license
  • Strong knowledge of primary medical care principles.
  • Demonstrated ability to collaborate effectively, including other physicians, facility staff, and office leadership
  • Working knowledge of hospice and palliative care, with emphasis on symptom management for terminal illness.
  • Strong leadership, communication, and organizational skills
  • (Preferred) Board certified/Fellowship in Hospice and Palliative Medicine

Key Responsibilities:

  • Provide ongoing education and training to inpatient facility staff in coordination with clinical leadership.
  • Participate in monthly leadership meetings with the Medical Director and inpatient clinical leadership. Participate in facility committees, meetings, and collaborative initiatives as requested.
  • Support and advise physicians and patient care teams to ensure high-quality care delivery.
  • Collaborate with attending physicians regarding patient prognosis and care planning.
  • Oversee and coordinate patient admissions, discharges, and transitions within inpatient settings.
  • Ensure continuity of care and effective handoffs between care teams during inpatient stays.
  • Promote best practices in hospice and palliative care across all inpatient settings.
  • Conduct periodic reviews of clinical care quality delivered by inpatient physicians.
  • Perform other responsibilities as assigned by the Medical Director, Executive Director, or their designees.
  • Make occasional on-site visits to hospitals for meetings, leadership needs, etc
