Qualifications

• Master's degree preferred for the role.
• 2 years of post-education experience required.
• Current NP or PA board-certified/licensed.

Responsibilities

• Provide patient care consultations and treatment for patients with serious illness with physician support.
• Participate in patient and family counseling and manage symptoms in collaboration with the interdisciplinary team.
• Take clinical calls for palliative care clients.
• Participate in Interdisciplinary Team meetings for care planning.
• Maintain accurate patient records.
