Job summary

SSM Health is seeking a Chaplain for the St. Agnes Hospital Home Care & Hospice in Fond du Lac, WI. The role provides spiritual care within the hospice team, focusing on assessment, counseling, and support for patients, families, and staff.

Qualifications

• Master's degree in Divinity or related field.
• One year hospice or palliative care chaplaincy experience.

Responsibilities

• Provides spiritual assessment to palliative care patients and families.
• Leads the plan of care for spiritual/emotional needs with the palliative team.
• Chooses pastoral care interventions and counseling techniques.
• Supports caregivers and families and addresses conflicts when needed.
• Provides grief counseling and bereavement support.
• Identifies cultural and faith traditions impacting care.
• Coordinates religious resources including sacraments when appropriate.
• Facilitates goals-of-care discussions with families.
• Educates fellows, residents, students and staff on spiritual care.
• Ensures timely documentation in the medical record and data collection.
