Job summary

Allay Hospice is seeking a Chaplain in Sheboygan, WI to provide emotional and spiritual support within end-of-life care. You will serve on an interdisciplinary team, delivering compassionate, culturally inclusive care to patients and families across diverse backgrounds.

Key responsibilities include conducting spiritual assessments, developing care plans, and coordinating memorials. The role emphasizes collaboration with community chaplains, education for the care team, and ongoing spiritual

Qualifications

• Graduate of an accredited theology or chaplaincy program (BD, MDiv or equivalent).
• Experience working with death and dying; hospice experience preferred.
• Comfort in an interfaith setting; respect for diverse beliefs and values.
• Must be a licensed driver with a reliable vehicle and insurance.
• Ability to collaborate with the care team and community clergy.

Responsibilities

• Conduct and ensure timely spiritual assessments for patients and families.
• Develop individualized care plans to address spiritual needs.
• Provide direct spiritual support to patients, families, and caregivers.
• Serve as liaison to community chaplains and spiritual counselors.
• Maintain accurate documentation of spiritual services and communications.
• Attend interdisciplinary meetings and contribute to care planning.
• Offer education and spiritual care guidance to the care team.
• Recruit and support community chaplains and volunteers.
• Coordinate and lead memorial or funeral services as requested.
• Organize periodic memorials for staff, volunteers and community partners.
• Assist in supervising spiritual care volunteers.
