Palliative Care Services in Cookeville, TN is seeking a Palliative Care Advanced Practice Provider (APP) to deliver expert, compassionate care across settings. This per diem role focuses on managing symptoms, guiding goals-of-care conversations, and aligning treatment with patient values.

As an integral member of the interdisciplinary team, you will develop individualized care plans, coordinate transitions, and support hospice referrals when appropriate.

Responsibilities

• Provide holistic assessments addressing physical, psychological, social, and spiritual needs.
• Guide patients and families through meaningful goals-of-care conversations to ensure treatment aligns with their values.
• Create and update personalized care plans in collaboration with patients, families, and the interdisciplinary team.
• Manage pain and other symptoms with evidence-based strategies.
• Support safe transitions of care by reconciling medications and deprescribing when appropriate.
• Initiate timely hospice referrals and ensure smooth handoffs when patients become eligible.
