Job summary

The University of Kansas Health System in Kansas City, MO is seeking an Ambulatory Clinic RN Care Coordinator for Palliative Medicine. This role supports physician clinics with patient care coordination, education, and ensuring the plan of care is understood by patients and families.

The RN will triage, manage patient flow, and coordinate with front desk, nursing staff, and physicians to optimize outcomes and maintain high-quality care throughout clinic sessions.

Qualifications

• High School Graduate.
• Bachelor Degree Nursing.
• 2 or more years of RN experience.
• Previous medical office experience (preferred).

Responsibilities

• Manage patient flow, triage, and direct patient care.
• Liaison of communication between front desk staff, nursing staff, physicians, and clinic manager.
• Manage the care of the patient population by developing expected patient outcomes, defining appropriate resource utilization, and working with the provider to set patient goals.
• Provide nursing support/coordination to the practice to optimize patient outcomes and to improve patient’s quality of life through clinical and educational programs and/or support services.
• Support patient self-management of disease.
• Coordinate continuity of patient care with patients and families following hospital admission, discharge, and ER visits.
