Job summary

Texas Health Care PLCC is seeking a motivated provider with pain management experience to evaluate and manage patients with chronic pain conditions. The role emphasizes comprehensive evaluations, development of individualized plans, and coordination with a multidisciplinary team.

Responsibilities include reviewing imaging and labs, educating patients, and maintaining thorough documentation. Office locations include Fort Worth and Granbury, with standard full-time hours 8:00am–5:00pm.

Qualifications

• Ability and interest in learning and implementing new processes, new tools, new activities.
• Diagnosing illnesses and injuries.
• Performing and ordering diagnostic tests and evaluating the results.
• Educating patients on living healthy lifestyles and managing their illnesses.
• Must have current state licensure.

Responsibilities

• Perform comprehensive new-patient and follow-up evaluations.
• Assess and diagnose acute and chronic pain conditions.
• Develop and implement individualized treatment plans.
• Manage patients with complex chronic pain conditions.
• Review diagnostic imaging, laboratory results, and medical records.
• Monitor treatment response and make appropriate adjustments to the plan of care.
• Educate patients regarding treatment options, medications, procedures, and expectations.
• Coordinate care with physicians, specialists, physical therapists, and other healthcare providers as appropriate.
• Maintain accurate, timely, and thorough clinical documentation.
• Work collaboratively with the physicians and clinical team to provide high-quality patient care.
