Responsibilities

• Assist with ISP assessments and planning documents.
• Conduct training activities to teach daily living skills.
• Record and report information to support updates to each individual’s support plan.
• Share observations with team to support coordinated care.
• Help maintain a safe, comfortable, and therapeutic living environment.
• Model appropriate behavior and uphold individual rights and dignity.
