Lifepoint Rehabilitation, at UK King's Daughter Medical Center in Ashland, KY, seeks an IRF-PAI data coordinator to support quality indicators and patient data entry. You will coordinate PPD data collection with the interdisciplinary team and ensure timely completion of IRF-PAI documentation within CMS timelines.

Qualifications

• Clinical degree and active license in OT, PT, RN, SLP or MSW.
• Experience with Quality Indicators (QIs) and CMS guidelines.
• Strong data-entry and medical-record documentation skills.
• Ability to meet CMS timelines for IRF-PAI admissions and discharges.

Responsibilities

• Coordinate PPD data collection with interdisciplinary team.
• Ensure completion of supportive documentation for Quality Indicators (QIs) in medical record
• Complete IRF-PAI admission and discharge entries within CMS time frames
• Summarize information from medical records onto IRF-PAI
• Enter admission PAI within four days of admission
• Enter discharge PAI within five days of discharge
• Input data into CMS-accepted program within designated time frame
• Update IRF-PAI log and maintain accuracy
• Verify patient location roster and log accuracy
• Ensure all IRF-PAI log columns are complete per discipline
• Assist in team education on QIs and documentation
• Complete QIs competency and assist others
• Other duties as assigned

Facility Name: UK King's Daughter Medical Center Setting: Acute Rehabilitation Unit FTE: Full-Time City/State: Ashland, KY

Lifepoint Rehabilitation is a leading provider of acute inpatient rehabilitation services with more than 300 hospital-based rehabilitation units, medical/surgical and outpatient therapy settings and more than 30 joint venture inpatient rehabilitation hospitals across the country. How you'll contribute

• Coordinate PPD data collection with interdisciplinary team
• Ensure completion of supportive documentation for Quality Indicators (QIs) in medical record
• Completes admission and discharge IRF-PAI within CMS designated time frames
• Collect appropriate information from medical record and summarize on IRF-PAI
• Ensure completion of admission PAI within four days of admission
• Ensure completion of discharge PAI within five days of discharge
• Inputs data into CMS accepted program within designated time frame
• Updates IRF-PAI log and maintains its accuracy
• Check patient location roster and verify accuracy of IRF-PAI log
• Ensure completion of all columns of IRF-PAI log with each discipline
• Assist in team education regarding Quality Indicators (QIs) and documentation
• Complete Quality Indicators (QIs) competency
• Assist in performing Quality Indicators (QIs) competencies with other team members
• Other duties as assigned
