Job summary

Northwell Health in New York is seeking an experienced registered nurse to deliver age-specific care using the nursing process and to collaborate with physicians and the health team in coordinating procedures and treatments on the patient care unit.

The role requires assessment of physiologic and psychosocial health, plan development, care implementation, and discharge planning, with leadership in coordinating patient care and directing activities of the unit team.

Qualifications

• Graduate from an accredited School of Nursing.
• An Associate’s Degree in Nursing is required.
• A Bachelor’s Degree in Nursing is preferred.
• Current NYS or CT RN license required.
• BLS required; additional certifications as needed.

Responsibilities

• Assess the patient’s physiologic health status.
• Collaborate with physicians and health team members in coordinating treatments.
• Identify and document patient outcome measures and plan of care.
• Formulate the plan of care and establish priorities to achieve outcomes.
• Implement the plan of care and ensure safe, effective treatments.
• Reassess and revise the plan as needed; ensure timely documentation.
• Coordinate discharge planning and referrals as appropriate.
• Evaluate care effectiveness and contribute to quality improvements.
• Provide education to patients and families about conditions and care plans.
• Support team members and delegate tasks appropriately.
• Perform related duties as required.
